Portland Family Practice has an innovative way to help our patients fully utilize their well adult exams.  Our pre-screening team will be contacting patients by phone prior to their scheduled Physicals and Well Women exams.  They will gather information; predetermine tests, vaccinations, medication refills, referrals, or orders that may be needed before or at the appointment.   Our goal is to provide the most complete competent health care in an efficient timely matter for our patients and clinical staff.
Heading the pre-screening team is Linda Teller RN.  Linda has 40 years of extensive nursing experience as a triage and assessment nurse in the fields of OB/GYN, surgery, Internal Medicine, Geriatrics, NeuroOtology and Family Practice. 
Expect a call from Linda or one of her experienced team member's- Vicki and Lucy; prior to your appointment.  We will do our best to make sure your questions and important issues will be addressed, as your care experience at PFP is very important to us.

H1N1 Influenza and Seasonal Influenza vaccine

Portland Family Practice has both of these type of vaccines in stock.
Please call scheduling for availability and eligibility

If your child has never had a flu vaccine in the past, and is under the age of 9,  they will
need to have two appointments, one month apart.

*Patients will need to check their insurance for coverage and sign a financial responsibility form or opted to pay a fee at the time of service.

Portland Family Practice would like to make our patients aware that the Oregon Public Health Division now has a dedicated Flu Hotline. Providing Oregonians with information on both H1N1 and seasonal flu symptoms, treatment and vaccinations. They will also provide medical triage for anyone that needs it.
Toll-free hotline- 1-800-978-3040  M-F from 8 am to 5 pm.
  Everyday at Portland Family Practice we receive numerous phone calls from patients with questions regarding H1N1
Typically patients want to know the symptoms of H1N1, if there is a test to diagnosis it, if there is medication to treat it, if there are other ways of treating it

Diabetic retinopathy is a condition that causes the tiny blood vessels in the back of the eye to become blocked, leak or even rupture. Early signs can be blurry vision, floating spots or poor night vision. Fortunately, like many of the complications of diabetes, the progression of retinopathy can be slowed or even stopped with early detection and appropriate treatment.
For this reason, Portland Family Practice has recently contracted with EyeTel Imaging to offer eye exams for our patients with diabetes. Now you can get your annual eye exam in the convenience of our office! 
The digital images are sent to Wilmer Eye Institute at John Hopkins Medical Center via the phone line after your appointment. Faxed or electronic reports are reported to your provider within 48 hours.
Your diabetic eye exam is billed to your insurance just like a medical appointment,
so call our clinic at (503) 233-6940 to schedule your exam.  If you do not have insurance, please contact our billing office for assistance.
