Diabetic care is very important to us here at Portland Family.  We have participated in the Oregon Diabetes Collaborative to improve and maintain a high standard of care.

We strive to provide the most complete diabetic care we can, including individual and group counseling sessions.  Please contact our office to schedule an appointment.  We encourage you to bring your husband, wife, partner or close friend to appointments, to assist you in reaching your goals.
Maintenance Schedule for Diabetes
Every 3-6 months:
  • HemoglobinA1C (3 month average of blood glucose)
Yearly:
  • Complete exam with your provider, including filament foot exam
  • Lab work to include: Cholesterol, Micro-Albumin, and Liver function.
  • Flu Vaccination
  • Dilated eye exam with Ophthalmologist
  • Goal setting/review

People with diabetes also need a one time vaccination of Pneumovax.  Frequency of follow-up appointments and lab testing will depend on your overall blood sugar control.

Nanis Gilmore, Registered Nurse and Certified Diabetes Educator is happy to meet with you and discuss your current practices-- what's working , what's not; and to help you explore issues affecting your diabetes management. Nanis has access to a wide variety of supplies and learning tools that may be useful to you, helping you champion your own health care.

Diabetes is tough.  When you have diabetes, you need to eat healthy food, stay active, control your weight, take your medicine, check your blood glucose (sugar), and have regular visits with your doctor, an ophthalmologist (eye doctor) and the dentist.  And that's on top of handling all the other things in life!  No wonder a person with diabetes can feel stressed out and afraid at times.
