Appointments
Patients are seen by appointment only.  When you make an appointment, please let the scheduler know which provider you wish to see, and briefly describe the problem so an appropriate amount of time can be scheduled.  If you have several problems to discuss, it is helpful to know in advance, to be able to address your questions thoroughly.  All of our PFP providers want to be able to meet your needs for unplanned health issues, and our nurse practitioners are a part of how we are able to see our patients urgently.  We have two Nurse Practitioners on staff, Carolyn Nowosielski and Joel Michels.
Carolyn is a dedicated and experienced healthcare provider who provides acute care for our PFP patients who need same day care.  Carolyn is able to diagnose, treat, and prescribe necessary medications and treatments for your unplanned health care issues.  She consults with our providers as needed.  Your follow up care is provided by your PCP.
Joel provides both acute care and long term care @  PFP.  His practice is open to patients who are looking to establish care with a PCP. Joel is able to diagnose and order treatments as other providers for acute and chronic problem.  Joel is a dedicated, experienced healthcare provider.  His practice provides care to all individuals of all ages.  He collaborates with specialists or other providers when needed.

If you are a new patient, or have new insurance, please arrive 15 minutes prior to your appointment and bring your insurance card, photo ID, any applicable co-pay, and the completed registration and HIPAA forms.  This will help us expedite your registration process.  It is also very helpful to have your medical records transferred to our clinic in advance.  You can use our release of information forms for this.  If your appointment is for your child to have a well child check, please complete the intake forms that where mailed to you or you can download from our forms page.


For routine visits, such as physical and well child exams, please call 6-8 weeks in advance so you may have a better selection of appointment times.  We save several appointments a day for urgent issues.  If you have an urgent issue please try to phone us as early in the day as possible, as same day appointments are usually booked very early.
If you are delayed or unable to keep an appointment, please call us as soon as possible.  This allows us to adjust our schedule to accommodate you and our other patients.  Our providers make every effort to stay on schedule to keep your waiting time to a minimum.  Sometimes we must provide urgent/emergency care or deliver a baby during office hours.  We understand the inconvenience this causes and thank you for the consideration you show to others at this time.
